Is Vantas Part B (and, if so, needs prior authorization?) or Part D?

My urologist is happy to do this, but to save his staff the time I am trying to find out if and how it is covered by Medicare. I find indirect confirmation on the internet it is Part B and needs prior authorization. I called Medicare and they were hopeless. Those who have done it--how does Medicare treat it?

It is an implant and should be covered under Part B.

B for sure. It is awesome. Much more even dispensing of medication and very simple once a year implant.
