As far as I know, the conventional wisdom is that you try Xtandi or Zytiga, regardless of any test results because it still might work.

In men who have not been exposed to either abiraterone or enzalutamide, it is less common to have AR-V7–positive CTCs. Moreover, these may be less predictive of a poorer response because 4 of 15 of these men did respond to abiraterone or enzalutamide in the current study. Therefore, in most cases, it is unlikely that the presence or absence of AR-V7–positive CTCs would play a major role in therapeutic decisions in this patient population.
I think this test might be useful for those who are starting to have progression on either Xtandi or Zytiga and considering switching to the other. If Zytiga no longer works and you have AR-V7, I think the chances are low that Xtandi would work and you might decide against trying it.
