Starting at a low dose is normal. The plan is to make sure you tolerate it and don't have too many severe side effects before it gets increased. There's no telling how soon or at what dose you'll start to notice benefits. Be patient and give it time. Try to take notes on any changes you notice. Make sure to communicate with your doctor regularly if you feel like nothing is changing. In the end, there may be a dose that's right for you or it may be a different medication. It will get there.
When you are receiving the dose that works for you, you should feel the effects about fifteen minutes after taking it. That is how it has worked for me.
The initial low dose is to make sure you don't have an adverse reaction to it. Most people notice the effect within half an hour or so. Its common to start a new patient at 30mg which should be noticeable and for a lot of people its very noticeable. The first time I took it, my mind went quiet for the first time in my life. It was as if I had spent my whole life in a roomful of people and the constant buzz of conversation and distractions - then suddenly, the room emptied. I found it much easier to stay engaged with tasks once I got engaged.
As others stated the low dose is more of a preliminary start to gage your bodies response to it. I started there but moved to 70 mg (highest) within 6 months. I have a very high tolerance for caffeine. Though this doesn't involve caffeine, it seems to go hand and hand with me needing more of and effective dose. Give it a month then your doctor would probably gradually increase the dosage.
