Dad has Stage 4 Prostate Cancer; rising PSA levels?
Hi everyone, I (25m) didn't realize how active this community was until recently! I was wondering if some people can help clear the air with what I'm seeing in my Dad (64). Last August (2019) my Dad was diagnosed with Stage 4 prostate cancer with bone metastases and a PSA level of 433 (Gleason score

High Testosterone after one month Leuprolide shot
My dad went to his first blood control one month after his first 3-months
Leuprolide
shot, but his Testosterone is still on normal levels (4.7 ng/ML equivalent to 470 ng/dl). His PSA come from more than 50 to 22, but I guess is only because Bicalutamide. His alkaline phosphatase is on 75.

Hi folks, are the Metastatic Hormone Sensitive Prostate Cancer (mHSPC) patients here, which, treatments are at least ADT2, taken Docetaxel?
So, ADT2:
Leuprolide
(Lupron, Eligard) and Abiraterone (Zytiga) for example, is typical combination for mHSPC patients. Although, the CHAARTED trial has determined - adding Docetaxel to ADT2 improves overall survival and prolong time to castrate resistance.

My father's metastatic cancer, a new challenge
Luckily the
Leuprolide
and Bicalutamide treatment are SOC and, therefore, given for free, but his MO suggested me to find a way to put him on Abiraterone treatment, since I know my father wouldnt agree to try Docexatel in this moment.

How many of you are using tE2 as your only form of ADT?
I imagine most of you know something about the old, synthetic, oral estrogen DES (diethylstilbestrol) that was used as ADT until the mid 80's when it was discontinued and replaced with Lupron (
leuprolide
).

Fit or unfit for chemo (Docetaxel)?
He is already on
Leuprolide
/Bicalutamida, and we are thinking about chemo or Abiraterone. Because of Covid19 his MO hasent seen him, just his test and results. She was concerned about two things before going for chemo: A) My father has lost weight during the last two years.

Stopping Casodex before stopping Lupron
I started Bicalutimide just prior to starting
Leuprolide
Acetate back in November. I have tolerated the ADT treatment fairly well with minimal side effects. A Decipher test was performed midway through this per my request and my Decipher score came back 0.71.

MONOTHERAPY FOR NEWLY DIAGNOSED PROSTATE CANCER
As of Dec 18, 2019, LHRH agonists available in the US include:
Leuprolide
(Lupron, Eligard), Goserelin (Zoladex), Histrelin (Vantas) and Triptorelin (Trelstar). An LHRH agonist keeps testicles from making T by blocking other hormones needed to make it.

Leuprolide: Eligard VS Lupron (1 month, 3 month, 6 months)
It is injected into fatty tissue as opposed to muscle and as a result maintains more stable levels of
leuprolide
acetate than do Lupron injections. The one disadvantage is that it appears almost no insurance covers Eligard.

Skip the injection, go straight to abiraterone
I was discussing abiraterone acetate (brand name Zytiga) with my MO and asked why it was always given with ADT. "I don't know, it's just always been done that way." This seemed odd to me. ADT usually involves a drug like Leuprorelin (Lupron) to trick the hormone system into shutting down testosterone
