Rising PSA after 7th infusion of Jevtana (Cabazitaxel)
Had early
docetaxel
chemo and then abi and some trial drugs. Had a test to look for genetic mutations a few months ago, none found.

Post Docetaxel treatment plan
My father completed his 6 rounds of
Docetaxel
and responded really well and tolerated it quite well at 75 years old with the exception of horrible taste changes.

Psa Jumps, but no signs of a failing sytem....
Because of the high volume (CHAARTED, STAMPEDE) ADT combined with 6 rounds of
docetaxel
, first treatment in July 2020. PSA level declined to 4.4, ALP 160. After the 6th treatment my PSA slightly increased to 5.0, ALP 170.

Update after 3rd Lu177
However, the latest F18 PSMA pet scan showed increased number and psma avidity of skeletal mets 😢 We have tried Zytiga,
Docetaxel
, back to Zytiga and latest is Lu177. Along with Xgeva and Eligard/Lupron all throughout since diagnosis in 2018. No genetic mutations when blood was tested 2yrs ago.

Suggestions on next step of therapy
I am concerned that if the
docetaxel
worsens my neuropathy I will lose what ambulation I currently possess. Is carbazitaxel ever approved without prior
docetaxel
use? It supposedly has less of the neuropathy side effects. Provenge is another option. Xofigo also, although my pain level is minimal.

PT-112 study
Ok, after many standard of care options (Lupron,
docetaxel
, Xtanti, Xofigo) and 2 clinical trials, this one looks like a good next step. My PSA has ballooned up to 175, but I am stills mostly asymtomatic with Mets in 4 bones.

PSA increase after docetaxel treatment
After the first
docetaxel
treatment in July, my PSA level reached a level of 14.7, ALP 170. After the 5th treatment my PSA decreased to 4.4., ALP 160. Unfortunately last week (4 weeks after the 6th and last cure) my PSA increased 0,6 nmg to 5 again. The ALP also rose slightly to 171.

chemo experiences
I will be starting chemo for the first time with carboplatin/
docetaxel
next week after recovering a bit from radiation therapy to shrink the prostate. Can others tell me of their experience with this treatment? They will also give me CSF (Zarxio). Age 69. Reside in the Pacific northwest.

"breaks the blood brain barrier" scares me
Docetaxel
scrambled my brain effectively but does not "break the blood brain barrier". Is this a cause for concern? All best, Baltha

Medication side effects or bone metastasis pain?
I had only mild pain with
docetaxel
mostly in my feet and shoulder but it went away within 4 months of completing chemotherapy.

High Dose Melatonin
I have been on 20 mgs per night for 10 years .Now upped it to 60 mgs x 4 times a day .This to offset
Docetaxel
for first time and result is no side affects from the Chemo .Will be doing scans in two weeks to see if metastasis has slowed ,currently in bone but no soft tissue involvement .

Not responding to Xtandi
Diagnosed 2008, RP same year, followed by salvage radiation in 2012, then Chemo (
docetaxel
) in 2016, then Lupron (for life), then bicalutamide(Casodex). In April, last year, switched out Casodex to Xtandi as psa rates were rising. Current psa Jan/21 7.24 with a doubling rate of 7.1 months.

2/3 of the way through docetaxel
Doc recommends staying the course on
Docetaxel
as ALP seems to indicate some progress. Hubby feels pretty good once he's through the first week after infusion. Pain is still manageable but never diminishes, only increases.

Update to Can't treat what we can't see
Following is from the After Visit Summary: We discussed a recommendation for enzalutamide at this point - while it may have a lower response rate compared to
docetaxel
, especially after progression on abiraterone therapy, the toxicity of
docetaxel
is significantly greater, especially in the context

To treat or not to treat: is it acceptable to avoid active therapies in advanced prostate cancer today?
This evidence of improved life expectancy does not support avoiding the addition of
docetaxel
or ARTAs to ACCEPTED MANUSCRIPT ADT in mHSPCa patients. 2.2 Disease-related factors for decision making Two main factors should be considered when defining the risk/benefit ratio of adding

How long can docetaxel work after treatment with abiraterone
Probably about to start
docetaxel
. I realize that everyone responds differently, but how long can I expect it to work after Zytiga has already failed?
