BRCA2 mutation and PARP inhibitor in the castration sensitive
I had been aiming for a more aggressive approach with long-term ADT perhaps transdermal estrogen, plus either Zytiga or
Docetaxel
. However, after reading the thread and some studies, I understand that using a PARP inhibitor would eliminate the need for all of these making my choice rather easy.

Neupogen Injection During Doxetaxil Treatments.
Good morning Warriors: I had my first
Docetaxel

Docetaxel/Taxotere stopped after 7th Infusion
My first chemo infusion was Dec 16, 2020, with the 21 day cadence. My last two blood tests prior to infusion showed my PSA jumping up again. I wasn't worried about my PSA rising as the fine gentlemen on this forum have stated that PSA may go up do to dying cells. MO told me "it's not working so we're

HOW GOOD IS DAROLUTAMIDE?
Abiraterone didn't work; having good luck with
Docetaxel
. Have metastases in lymph nodes; bones clear so far.
Docetaxel
has run its course. Anti-receptor is next. MO wants old standby, Enzalutamide. But I've read highly complimentary stuff about new AR, Darolutamide.

BiTE Clinical Trial
Treatment history: He started off immediately on Lupron and had
Docetaxel
, which helped a great deal. He switched ADT to Apalutamide + Firmagon about a year in, which he’s still on now, as well as Zometa every 3 months. PSA is now at 2.1 and slightly creeping up.

First Follow Up on MyTestosterone Trial
Elected 6 cycles of early
Docetaxel
, then SRT to prostate bed only, all in 2007. Intolerant/allergic to leuprolide so I did alternative ADT after BCR with bicalutamide 50mg plus dutasteride for 5 years before it failed. In 2019 I had Ga-68-PSMA scan showing only two PLNs in pelvis on left side.

What's next after my failure of ARV-110 trial
Am I missing out on having a proven effective treatment of Carboplatin + Taxane/
Docetaxel
chemo, since I am BRCA2+, by going on to another trial instead? Next trial is possibly AC225 + Xtandi + Keytruda combo therapy.

Docetaxel vs. Cabazitaxel--effectiveness?
If he's progressing after
docetaxel
what are the chances cabazitaxel will help? He's really reluctant to do more chemo as he has a lot of nausea and vomiting. Anyone here had success with cabazitaxel after failure of
docetaxel
?

When to do a CT scan?
Here’s the recent background ( also see profile): Back in November I posted that my husband had completed 6 rounds of chemo (
docetaxel
)—concurrently he was receiving Eligard injections every 3 mos. and his PSA #s were coming down incrementally.

NICE not playing nice again.
Though clinical trial evidence showed that study subjects taking Lynparza had improved progression-free survival and overall survival, NICE said that there is no direct comparison of Lynparza to the current standard of care for metastatic prostate cancer, which includes chemotherapy drugs

Stage 4 initial recommendation Docetaxel and ADT
MD Anderson recommends
docetaxel
and ADT with primary site radiation after. I see there's some data of maintaining better control with early chemo, but wanted to know if anyone else has experienced it. My urologist recommended ADT and IMRT to all sites.

Total combat
At my current state of knowledge this means combining ADT (transdermal estrogen) with chemo (Jevtana or
Docetaxel
) as well as Carboplatin, plus second-line hormonal therapy perhaps Zytiga plus a PARP inhibitor since I have a BRCA2 mutation. Possibly low-dose Bicalutamid too.
