Not sure I am following.....the Gleason is based on the highest two patterns within each core, not the sum or percentage of patterns taken as an aggregate.
You are misunderstanding. Your Gleason score is the core with the highest Gleason score. A core is GS 3+4 if less than 50% of that core is pattern 4; GS 4+3 if more than 50% (and the rest of it is pattern 3).
Treatments are based on the highest Gleason, total of bit numbers. 3+4 is not the same as 4+3. The first number relates to the differentation from normal. 4 is a bit worse than 3. The second number is the less prevalent of the PCA cells and while 4. Is worse the 3 being the second number just shows some are there
