SBRT vs EBRT

After lots of research, have decided against surgery and opted for RT. Thank you to everyone in community for views and opinions.

Went to my RO to determine best course of treatment for my PSA 8, 4+3 PCa (single localized tumor). We decided on SBRT + ADT. Found out today that the SBRT supplier has declared bankruptcy and the hospital can no longer offer SBRT. Since my PSA went from 5.5 to 8 in 10 months, my RO felt EBRT + ADT + 1x seeds would be a better option anyway.

Anyone in same boat? Other options? Opinions?

I expect you will be fine. I did 44 sessions of IMRT (without BT) and it not that big a deal other than the time commitment. Everything went smoothly

ADT sucks and your main decision will be how long to stay on it

My understanding is that Viewray MRIdian will no longer be supported as of October 25. However, SBRT can be given on a number of devices- it only requires fiducials. I was treated with RapidArc and many are treated with CyberKnife.

HDR brachytherapy has fewer urinary side effects than LDR brachytherapy. Where are you located?

Hi, Located in Oregon. I have read that SBRT supplier, Sorrento, is going into bankruptcy.

You can try OHSU. I know you can get it from Robert Meiers at Swedish Health in Seattle.

Will look into this. Am a bit 'tied' to Providence Medicare.

I'm sure Providence offers SBRT, plus HDRB....

Perhaps there are more ROs that could do HDRB for you......you've probably already researched this? Is that plan an HMO or PPO?

Stephen Bader in Portland provides SBRT. I had a consult with him last month and he has quite a bit of experience.

He's my RO too and the one that told me about SBRT bankruptcy.

I have also decided to go the RT route when the time comes, I am a 3+3 and have decided on AS for the time being. I have looked into SBRT and HDR Brachytherapy and if I were to get treatment today I would likely go the HDR route. I have heard very good things about Dr. Jing Zeng.

He suggested EBRT with ADT + needles at end.

My favorite HDR brachytherapist is Mitch Kamrava at Cedars-Sinai

I have seen studies of an EBRT protocol that includes SIB......simultaneous integrated boost I believe. Perhaps results comparable EBRT + brachy, and I think SE probabilities were very favorable. You might ask about availability as an option. Google and pubmed will give you a number of recent studies......also TA has an article on his blog.

i had SBRT and it was a breeze. The ADT is a hurricane. Start hitting the gym now, lift weights. I bet you can find another practioner who will do SBRT. It only takes five sessions over two weeks.you could fly to Los Angeles, Seattle, of SF, stay in a holiday inn and do it.

wonder what UCLA is going to do. They had a Viewray. I was treated there last year. MRI guided SBRT. Is too bad because I think it’s a great technology

I agree. Smoothest treatment imaginable and great results so far. I’m surprised someone doesn’t buy the assets.

Read on line that Sorrento was offered $1billion buyout but decided to 'weather the storm'. Got a new CEO but things didn't work out.
