Baby Skin Issue on Bottom and genitals really worried

hi all, am really worried about our 6 month olds bottom and genitals rash/skin issue. We had the doctors on Tuesday and they prescribed a fungal cream but it’s got lots worse. To caveat I do have another appointment tomorrow and I’m going to ask for a dermatologist appointment referral but I want to see if anyone has had anything similar? I’m panicking it’s like a skin eating infection as it’s so raw! They are like little geographical craters almost on his bottom and his genitals are just red raw skin! Only posting bottom picture for modesty. Has anyone had anything like this on their little one? Panicked mum here. Thanks x

Oh my lovely that does look sore! So sorry baby is going through this as I know it’s hard when you know they are not well. I haven’t had this with my boy but I couldn’t not reply. I’m guessing there has been no changes to nappies, creams, washing powders or bath stuff. Thinking could it be an allergy. My friends baby had really bad nappy rash at one point where it was weeping and a cream sorted it so fingers crossed it something similar. You are obviously doing all the right things and I really hope the docs can prescribe something to soothe the skin. Lots of cuddles to you both xxx

Thanks for replying, honestly I’m so worried. Had the fungal cream for doctor and it’s got worse with it after a week or so. Not changed a single thing in what we do/use. It just looks so red and raw and really weird unlike what I’d call ‘normal’ nappy rash. It doesn’t seem to bother him which is good and no temp or anything but obviously don’t want it spiralling into something serious if it isn’t treated swiftly x

Thank you for thinking of us! Honestly I’m totally surprised but before I collected the prescription and antibiotics from the chemist I just put tons of Sudocrem on the red parts just to tide him over until I collected them, and honestly within a few hours it had totally gone down tons! And I mean tons! Which is so weird as I’ve previously used Sudo for normal nappy rash and it doesn’t work, but on this it seems to have done the trick! I’m so surprised. Really strange! I’ll take a picture tomorrow to show you because I think it’s a really interesting case study. I’m baffled! X

It could be that sudocrem is simply not so good for non-infected or fungal nappy rash, but better for bacterial. I don’t know how effective benzyl alcohol is for Candida for example but it should be effective against most bacteria as long as the infection is superficial. So pleased it’s looking better-it’s awful when you feel it’s deep and glowing, must be so sore for them xx

good morning, my little girl had this, to be honest it got even worse open seeping wounds… I was also soo worried and we tried all the different creams. It turned out she had a milk allergy. As soon as I stopped eating dairy it cleared up completely within a week ❤️ she’s three and half now and outgrown the allergy. It may not be that for you but just wanted to say what was for us. It’s so upsetting when there is something wrong and you are tearing your hair out trying to fix it. Hope he feels better soon ❤️ when she did get a rash once we’d eliminated the dairy we would do bepanthen then metanium over the top which seemed to wirk well. xxx

Thanks for your reply. Glad your little girls raah rounds resolved and you got to the bottom of it. Our son has only just started solids and is on formula I don’t think it’s an allergy to anything. This started 2 weeks ago and it’s just gradually got worse, he only started solids about 3 days ago as he’s only just literally turned 6 months. So at least I know it’s not food related at the moment. It’s frustrating as I don’t feel the doctors are sometimes that clued up on baby skin conditions that’s why I might ask to be reffered to a dermatologist x

yes completely agree re the doctors not being so clued up on it. Fingers crossed you can get a referral and they may have some more answers and options to try work it out. Sending you all the good vibes for getting it fixed ❤️

It may be bacterial, which is why the fungal cream isn’t working. I would think your GP would take a swab for culture at this point. If he’s just started solids, or teething, it’s possible his microbiome and faecal pH has shifted, which can cause pretty nasty nappy rash. I swear by Metaniun with Sudocrem over the top as a barrier (we have had some really nasty nappy rash in the past similar to this) , but I would definitely wait to see the GP before covering him in lots of creams. Hope you get on top of it soon. Also, don’t forget you can use Calpol for the discomfort, it’s not just for colds. Once my boy was about 18 months he could tell us verbally he was sore when he had a nasty rash over bum and scrotum (nursery aren’t always fastest at changing him and he’s very sensitive to delayed changes) and I felt a bit bad not using Calpol when he was smaller when at its worst Xx

Thanks I’ve just had a video call with my GP and they’ve prescribed antibiotics as it may be bacterial they said also, so thanks for flagging. They’re also prescribing a mild steroid cream to put on it also and suggested using sudo in between changes of the steroid cream to barrier it. I really didn’t want to give antibiotics to a 6 month old but I guess if it’s bacterial then hopefully that will help before it gets worse. She said that crater edge often indicates a bacterial infection or the fungal cream would have helped by now. I think you’re right all it takes is for a nasty poo to get into a normal nappy rash before it changes into something like this x

We had so many episodes of mild to moderate rash, and a few really bad ones, despite our best efforts when just using Sudocrem that we started using Metanium and Sudocrem every change and it made a big difference. I think some kids are just really prone to it no matter what you do. Xx

This worked for my kids and grandkids: frequent nappy changes to prevent corrosive mix of urine and faeces, leave nappy off as much as possible, apply suitable cream (Sudacrem, Savlon). If the problem is nappy rash you should get quick results.

Can make Magic Butt Cream! It’s a tube of each: triple antibiotic/yeast treatment (trade name Nystatin)/hydrocortisone/ zinc oxide. Generic just fine. Mix well and use to cover area(s). Let baby be without diaper for a little while. Will see improvement within hours. Was RN in nursing home and we made our own
