Mat b form
If you have been employed by your employer for long enough to claim statutory maternity pay then you give them your form and they will arrange your maternity pay. If you don't qualify then you need to apply for maternity allowance. You'll need to do this as soon as you qualify as they're is a huge backlog on maternity allowance and people are only receiving it about 14 weeks after they claim.
If your partner is taking paternity leave he needs to write or speak to his employer, to let them know he intends on taking it.
Or you can apply for shared parental leave if that's what you're planning on.
It's all set out on the government website what you might be entitled to and how to claim

I've worked for them for 2 years they have asked for the form but I think they want original
They can have the original to take a copy from and then return it to you. I needed mine for a new job who wanted proof of pregnancy to explain my career gap so you need to either make sure you keep a copy for yourself or get work to give you back the original. You should qualify for statutory maternity allowance so would not have to apply for maternity allowance. Work should just sort this out for you and you would just get paid as you normally do. You would also get 10 keeping in touch days that you can do and get paid for, you will accumulate holiday and could be able to do shared parental leave with your husband. I would really recommend reading the government website so you get a good understanding of what you're entitled too.
Ok never heard of shared parental leave how does that work
It's all on the government website as well as an eligibility checker. I didn't qualify for it so have no first hand experience of it. gov.uk/shared-parental-leav...
If you speak with your midwife, they issue the forms, but it’ll depend if you qualify for it first ☺️
Your OH needs to speak with their employer to see if they need a copy sending across (more than likely)
I have kept my own form, took a photocopy just incase I loose it and a copy emailed to my employer and also my OHs employer
Previously I sent the original toy employer. Needed a copy for husband to give to his work. This time due to covid work only wanted a copy so sent a picture of it in and they were happy with that. Ask your work what they want but make sure to keep a copy.
