Hello

I also started on 15mg(...the starting dose is based on your body weight so therefore varies from person to person). It took 10 days before it really kicked in and I became painfree.....I remember phoning the consultant after 7 days in a panic..as they appeared not to be working but he was right and said it would work within 14 days........It was like a miracle when they did...

It is actually quite unusual for dose to be based on weight - other than maximum dose for GCA. In recent years, 15-20mg has been presented as a starting dose irrespective of weight and the last recommendations in 2015 advised "the lowest effective dose in the range 12.5 to 25mg/day" with no reference to weight. In fact the only real discussion of weight I have seen was in an Italian study where they tried 12.5mg as the starting dose. They found that 75% of patients achieved a good result within a month - with small women faring better than large men. Weight probably DOES have a role to play but even smaller people sometimes need more pred at the outset. I also know a few people who have had PMR more than once and more than one has said they did better the time they had a higher starting dose, usually 30mg which was common a couple of decades ago. That certainly seems to be the case in GCA.

Hi Moorfield. Welcome. Crikey I'm glad my Rheumy just gave me a basic starting dose! Although I am five feet nothing in height I make up for it with a wide girth! If my Rheumy had based my dose on weight I would have been on a ton of pred! 😂😂😂😂🙈🙈🙈🙈

I know they base the Pred Dose for Children on their weight, 1mg per 1Kilo of body weight, will l ever forget? And l had to weigh Martin regularly in case of an Emergency but never heard of it for Adults....

Oh, that's good to know!

I'm on my 4th day of 15mg and the night pains and morning pains are much worse than they were before I started. I'm getting despondent and worried that Pred isn't the right drug for me, or I should be taking 20..., so I guess I have to grin and bear it!

Yes, I saw a study early on in my journey that said low weight patients could be started on 12.5 mg. (of course I can’t find it now to cite for you—maybe Mayo?). I showed it to my doctor...this was after I was having such horrible side effects from 20 mg. Pred. I’ve always thought it might have been a viable initial starting treatment for me, sorry I can’t find it.

No - Italian, Genoa and Pavia. Just saying 12.5mg is enough - but heavier patients need more. Those patients must have been very patient - waiting up to a month for a good result!
