The most common first symptom is food getting stuck after you swallow it. If an esophageal tumour had grown large enough to cause pain i would have thought that the swallowing issues would already have happened. Usually a gastroscopy is the first test done to visually check the esophagus.

The complications are the restriction of the esophagus, and also the usual cancer complications where the tumour can spread beyond the original location through the esophageal wall or to distant sites.
