Need information for continuing care

Hi,

I have two sons with autism and severe learning disabilities age 10 and 11 years old. I have heard of the NHS continuing care and we are in the process of asking for an assessment. Has anyone been through it with a child?. We have heard that it is very difficult to get it and a lot of people do not know it, even the gp did not know what it was

I would like to know how it is done and how long does it take.
