You should consult your doctor. You can only take medication just before your meal. You cannot take medicine if you are fasting after taking the medicine.Your doctor will advise you to modify your medicine timings to suit your fasting period.

You can consult your doctor, however my experience is that fasting brings down the blood sugar level. You can take your medication after breaking fast or before if you are somebody who eats in the morning before fasting begins.

My little interaction with some learned Islamics is that, you do not have to fast but rather do "chiariwa" - charity/donation to the poor and needy for 30 days as enshrined in the Holy Koran. Diabetics in general are not expected to perform ant spiritual fasting.
