Anyone in a similar situation OH occasionally daytime, High BP early mornings.

Even though my husband was diagnosed with orthostatic hypotension he wakes up with high blood pressure. They took him off the beta blocker. We do all the things suggested, raising his head for sleep. And all along we were drinking gobs of water before he got up in the morning thinking he had the OH. Stopped that a few weeks ago since we noticed his BP systolic was quite high. Just wondering if anyone in the same boat and if you came up with a treatment plan. Confused and not much help from Doctors.

My MS says better high than low because low causes dizziness and falls. That depends on how high. She expressed no concern at 150/85.

His cardiologist said the same. But he is reading 186/88 this morning @ 9 am. Who knows how high when he is sleeping. Dropped to 157/72 at 11:30 am after his 9 am L Dopa.

He takes his meds at 9, 12, 3, 6 and 9pm.

12 pm with 1 mg ropinrole, 6pm w/1mg ropinrole.

Thinking using the Parkinsons meds to control the higher BP during late night.

The only time I am seeing 100/60 is in the afternoons now and not all the time. Probably need to do the 24 hour ambulatory. 🥺

Corrected, Well mine is higher in afternoon:

I am having high BP too. I will be interested in what others say.

He drops as the day wears on but I think because post Parkinson Meds or he would stay high. Occasionally the really low readings but not syncope level anymore since weaned off the beta blocker he was on for years. Lowest for months post BB has been 106/58 with no symptoms. Has to be away to control both OH and what seems to be supine hypertension safely. He sleeps with his head raised with an adjustable bed too. No way is his lower during sleep. Only way to find out is the 24 hour monitor even if a 24 hr stay in the hospital i guess. Onward!

My BP became erratic and improved after blood tests showed low salt and protein. Eat more of both, I think it is OK now.

Yes but i doubt there is treatment that lowers BP at night only. So i decided that trying to get answers is a lost cause. I decided to put my BP machine away and live instead. After all this is just one problem……..

One thing you could try is taking ldopa in the middle of the night but sleep is important so I would only take it if i awoke during the night.

Neurogenic orthostatic hypotension and supine hypertension are common manifestations of cardiovascular dysautonomia in Parkinson's disease. Because these disorders are haemodynamic opposites, improvement in one might be achieved at the expense of worsening of the other.

I thought about around the clock ldopa too. I know what you mean. Sometimes we need a break from the BP monitor. Thank you,

Have you tried the Pyridostigmine? Seems the least to worry about. Thank you for the article.

No I have not tried Pyridostigmine. It is not prescribed for PD is it?? What symptoms would I have it for?

It is one of the Meds they use to treat OH, supposedly doesn’t cause the high BP. Still scared!

It is made for myasthenia gravis I think and underactive bladder. Our problem is overactive bladder! Not used in my country for PD.

True the getting up at night to potty. I thought it was the Lupron shots he takes for Prostate cancer. Tomorrow we are splitting his 25/100 LDopa and taking it every two hours. Starting at 7 am, ending at 1am before he goes to bed. He sleeps until noon so we will adjust. Still going to stay on the Ropinirole 1 mg twice a day. He takes it now at 12:00pm and 6 pm with a full L LDopa. Will take it with the 1/2 dose LDopa at 11 am and 5 pm. I will ask the Neuro Nurse about the mild Prosom maybe start with 1/2 mg to start since he does talk and yell during the early morning hours. That alone can raise BP I think anyway. Just a trial. Hope to balance his high BP. His OH is almost non existent since of the Beta Blocker but occasionally he gets some OH. Like another member mentioned we might see changes over time since he was on the Beta Blocker years before his PWP started. And to add Lupron shots cause BP problems. Mostly HBP. Ugh, trying to keep this man with me awhile longer.🥰

Gazey, ( you are in Aussie or UK right?) If you are trying something new may I suggest you give it a couple of weeks to decide if it is working unless he is finding the adjustment intolerable. BTW Madopar comes in 50/12.5

I am always adjusting my meds. I can't get it right, I'm always over or under dosed. I stopped one amantidine at neuros suggestion and lasted 3 days then had to go back on to make life bearable.

We are in the Southeast US. That is what he just said, lol. No jumping around, because we recently tried adding ubiquinol back to see. The LDopa is scored but I need to be sure it will be the 1/2 dose.

Have you tried a Melatonin brand that helped you? When his OH was bad I wanted to try but read it lowers BP and I was afraid he would wake for the bathroom and fall. I don’t always hear him if in a coma. Maybe now since off Beta Blocker and night time we can try it. How many miligrams do you take?

I don’t think chartrist has PD but he is interested and especially in melatonin.

I can not be an example for the study results because they used low dose controlled release melatonin. I use high dose melatonin (120 mg+/night) which is in no way comparable to the study product. I use melatonin for entirely different reasons and control my high blood pressure with an herbal multi supplement that keeps my blood pressure tightly in the normal range with once every third day dosing. I also do not have PD. I was merely pointing the study out in case you were interested in looking into it.

How long since he came off the beta blocker? It can take months sometime years for the blood pressure to stabilise, especially if he’s been on them for a long time. I understand this is because, while on the beta blockers, the body physically adapts to squeeze as much adrenalin as possible and these changes to transporters and receptors have to be slowly reversed…. Search for ‘beta blocker withdrawal’ on HU forums, lots of valuable info….

I take siberian ginseng. It seems to lower the high and elevate the low...do a bit of reading on it

I read somewhere they researched and its a myth. Im looking for the page because i also read that it balances it...cant find it. Anyway mine was slighly high...now it seems to have balanced.

My husband has the same problem. Our wonderful Pharmacologist came up with a solution. He takes a blood pressure tablet in the evening, which keeps his blood pressure under control over night when it normally goes up. By morning it’s worn off when his blood pressure tends to be low.

That has been working reasonably well UNTIL now. Recently his high blood pressure seems to have stabilised. It’s much lower in the evening. I’m wondering if it’s the high dose vitamin C he’s been taking for constipation (as recommmended by park_bear). Some research suggests vitamin C lowers BP in people with high blood pressure (but not in people with normal BP). He is having a BP monitor next week to see what’s happening over a 24 hr period. It will be interesting see what happens over night.

He has also developed heart problems and has been put on diuretics, which complicates things some.

That is what I thought would work. Crazy thing though is his BP is highest in the morning. He gets the OH during the day occasionally and after he sits awhile watching tv and gets up. Interesting about the Vit C. I knew it had laxative effects at the correct dose. The 24 hr check is what I am asking the cardiologist for too.

Or it may just clarify that this is happening. Let us know if you find something that works on night time BP only. What meds for BP is your husband on gwendoline?

Hikoi you mentioned waking up taking levdopa? Long read, a-lot of work but makes the most sense to me.

Hi Hikoi, my husband takes a blood pressure tablet in the late afternoon, based on when his blood pressure starts to rise. By morning, it’s worn off, which is when his blood pressure is low. The medication is Candesartan Cilexetil 8mg.

Something has changed in recent times, as his blood pressure seems to have stabilised. My husband is now taking a diuretic for his heart and high dose Vitamin C for constipation. I read some research that stated that Vitamin C lowered BP in the trials, but only where high BP existed, not in participators whose blood pressure was normal. The diuretic can also lower BP. He is using a 24hr BP monitor this coming week to see what’s happening during the night. Life is getting more and more complicated.

That medicine sounds familiar. It was in the article from China. I am going to read up on it. First want to see if this Dopa schedule stabilizes him. Seems the meds used to help PWP can cause the exact problems they are having if not used properly. IDK🥴
