What does the liver is diffusely hyperechoic mean on US? Would they have noted a lesion if found?

It just means that diffuse (throughout a wide area) of the liver there are more dense (hyperechoic) areas which have been picked up on the scan. These dense areas can be areas of fatty build up and/or fibrous tissue.

Hopefully your doctor will be in touch to discuss this further and carry out any additional tests that are needed.

Thank you, Katie. If I had a lesion, it would have been noted on the US, correct?

If there was a lesion of any concern the report would usually state a lesion of unknown type or similar and suggest follow up MRI or similar - at least that's what my hubbies ones have said when he has had lesions.

I've had many ultrasounds and they always note two small cysts that are of "no concern".
