What does diffusely echogenic heterogeneous mildly coarse in texture without focal defect mean on an ultrasound

This is a description and not a diagnosis and is basically saying your liver isn't smooth as it should be - it is irregular and course. Why this would be needs to be looked into further to determine whether it's fatty liver or cirrhosis. None of us can say, your doctor will need to look into it further.

Your doctor may be able to interpret the scan from the information they already have - none of us are qualified or allowed to tell you what a scan means and in this case all it is telling you is the liver isn't smooth as it should be, it is lumpy and bumpy and coarse.

If the scan still doesn't provide them sufficient information there are further tests that can be done - things like the ELF (Enhanced Liver Fibrosis) blood test or they might send you for a fibroscan which can give you a CAP figure (tell you if it's fatty liver) and a kPa score (which tells them how dense the liver tissue is).
