Fat in liver and Mechanism of inflammation & cirrhosis

it is a bit complicated question , i just wonder about cirrhosis Mechanism ! is it necessary that a fat accumulation in liver is must a condition prior of any type of inflammation or cirrhosis ?! regardless of the cause of inflammation , for example consuming a lot of alcohol over the time lead to the accumulation of the fat in the liver and cause NASH and cirrhosis in later stage , some drugs cause cirrhosis because they accumulate fat in the liver , does viral or other cause like PBC ,PSC , Autoimmune Hepatitis and in specific Cryptogenic Cirrhosis which is my case

if my CAP score recently is normal and (before was high or above normal )does this means no more inflammation is ongoing ?

CAP specifically relates to the level of fat in the liver and is nothing to do with inflammation. If you had a blood test and your ALT and AST levels were also normal then it would indicate no ongoing inflammation either.

In fatty liver disease and alcohol related liver disease the liver goes through a number of stages before it reaches fibrosis/cirrhosis ( because you can't post pictures in replies I have added this graphic to a new post the link to which is - this for future readers who might be looking back - healthunlocked.com/britishl.... ).

In some of the other causes of liver disease you mention there is no fatty build up prior to liver damage - in the auto immune conditions the bodies own immune system attacks particular cells within the liver whether it be the bile duct cells or other liver cells - a liver biopsy looking at the liver on a cellular level tells where the inflammation is occurring (i.e. where the body is fighting off attack) and helps to diagnose what has caused/is causing live damage.

NASH is Non-Alcoholic SteatoHepatitis and is where the liver is fatty and inflamed at the same time. Definition of hepatitis is purely liver inflammation. So alcohol related liver disease doesn't include NASH.

What actually causes fibrosis is the liver attempting to protect it's cells - it throws out a web of protective fibres to protect the liver cells but as more and more of this fibrous tissue builds up in the liver it actually starts to squash the liver cells and puts them out of shape and as a result their supplies are cut off and the liver cells die leading to scarring and a mis-shapen, shrivelled and shrunken cirrhotic liver.
