New initiative with Care City to support outcomes in NE London

HealthUnlocked have today launched an exciting new digital health initiative with Care City to support people in NE London, in particular those with dementia.

By using digital technology such as online communities within HealthUnlocked, we will rethink management of a range of conditions. Working with Care City we will embed into GP and health care provider IT systems to provide individuals with a social prescription based on a person's long term condition.

The initiative aims to improve outcomes, reduce demand on the NHS and improve engagement with people’s own health.
