Can someone give me an advice...Am I needing for an lung operation?

THORACIC CT EXAMINATION In the examination performed by giving intravenous contrast material; Tracing and calibration of mediastinal vascular structures are normal. Calcified-non-calcified plaques with irregular surfaces were observed in the aorta. Heart chambers are within normal limits. The trachea, the carina, and the main brooches and branches are open. No lymph node in pathological size and appearance was observed in mediastinal lymph node stations. Bilateral pleural surfaces are normal. Pleural effusion was not observed. There was no active infiltration or a significant mass lesion in the lung areas. Panlobular emphysema that caused parenchymal destruction of all lobes in both lungs was observed. Focal fibrotic recessions are observed in the left lung upper lobe apico posterior segment lateral. Degenerative changes are observed in the bone structures within the cross-section. Abdominal organs in cross section; A cortical cyst of 45 mm in diameter was observed in the left kidney.

Nobody on here is qualified to give you a diagnosis based on your recent Ct scan.However in my humble and non qualified opinion,apart from obvious emphysema type damage to your lung parenchyma which is bsically the alveoli where gases are transfered,no tumours or lesions have been noted that would require immediate intervention.

The focal fibrotic recession in the upper left lobe may need to be further investigated?or it could simply be scarring from an infection?

Please speak to your Physician/doctor for a full diagnosis.

P.s a small cyst within your kidney is noted which i am sure your doctor will discuss with you.

Thank's alot for your kindly reply...My doctor adviced me to conteneu on some exercises and drugs as Spriva and Extraair..I replaid for bronchoscopy operation, but they said no urgent need for this operation...I am having a seriose attack when I am spending some effort or going from a warm place to outside when the weather is cold.

You could probably benefit from a lung function test and perhaps some structured pulmonary rehabilitation,this will help get you into a sensible regime of exercise but also advise on certain things you can do whilst exercising and everyday life to help with your breathing and there should also be an element of educational learning of how our lungs work and how we can keep them functioning to the best of their ability.Good luck for the future.

Thank you very much ... I have been examined by many specialist doctors since 2017.The answer which I received is almost the same .. They said that there is no treatment for emphysema, but operations such as lung transplantation or coil insertion can be performed in very heavy cases. I did respiratory function test and my FEV1turned out to be less than 30%. But in spite of that, when I don't spend an effort , my situation is fine and I can fulfill my daily functions.

They sound like they are on top of things,i actually had valves fitted but they failed to achieve the desired results,given your 30% values at the moment,and if you are meet the eligability requirements you might like to consider a similar option in the future and it is well worth discussing any options for a better quality of life in the future.Lung transplantations are rarely undertaken on patients who do not have a potentially terminal diagnosis and best left i think for the younger generation,though of course others may not agree with that statement.

Thanks again...Yes ...Valves fitting is the operation which I wanted to did , but they said you are not meeting eligability requirements in spite of my FEV1 valeu is lower than 30%. I just want to know if your condistıon improved after fitting the valves and if there are any risks after fitting the valves. Are you getting any difficultieis when you spend any efforts? Meeting the eligability is more to do with the type of emphysema you have and the structure of your lungs as opposed to your Fev1 levels.The risks post procedure are minimal and could be explained to you by your physician,they mainly relate to pnuemothorax complications.

Unfortunately i gained no benefits and the diseased part of my lung did not achieve something called atlelectasis which is basically when the diseased area withers away through lack of oxygen thus making it easier to expel trapped Co2.

My lung function had deteriorated further and at less than 20% now i believe i am heavily dependent on O2 for daily living and exertion but otherwise i lead a reasonably healthy life with no pain or anxiety.
