I think when your lungs crackle it means you may have some fluid in it or something causing it to be less able to inflate and deflate easily as you breath in and out when you are lying down. If you sleep on your left side then it may be only on the left side?

When my husband had a crackle in his left lung it was heard when the nurse and the doctor put the stethoscope on his back.
As has been mentioned, orthopnea is shortness of breath while lying flat. It is usually a symptom of early congestive heart failure with fluid congestion building up in the lungs. The heart has to pump harder when you lie flat and fluid that is reabsorbed from tissues enter the circulation. Sitting up offers some relief. Rales or crackles, heard when examining the lungs with a stethoscope, are early signs of fluid accumulating in the lungs but can also be heard with scarring or pneumonia. Treatment of incipient congestive heart failure includes salt avoidance, use of diuretics, sleeping with the chest higher than the abdomen and other meds deemed advisable by your treating physician who knows your case well.
