Can clopidogrel irritate a hiatus hernia? I've been taking it for a year, following what medics think was a TIA, and have a lot of chest pain and breathlessness.

If you have chest pains you should seek advice from a health professional such as a GP to decide whether you are suffering from a cardio related condition such as angina, or gastro such as reflux disease or a hiatius hernia, and that will take into account any medication you are taking. But if your chest pains become significant you should attend A&E.
I doubt it, and if you have had a TIA it is an early warning sign for a stroke, so I suggest you go to A and E next time you get the pains and contact your GP asap.
