Pleased with the following review, but why LVSD cause unclear?: " When we last caught up, the echo then demonstrated a deterioration in cardiac function along with high velocities across the aortic valve prosthesis. I initiated him on bisoprolol in addition to pre-existing ramipril and this has had good effect. He has started to become more active and lost 10 kgs, and his exercise tolerance and breathing has improved significantly. There are no features of heart failure. This corresponds to the CMR which has demonstrated an improvement to the LV systolic function which is now just mildly impaired, and the aortic valve gradients are estimated around the 2.5 m/sec mark.
The aetiology of LV function deterioration remains unclear, but it is reassuring that things are improving. I will increase the bisoprolol to 2.5mg bd and we will have a repeat TTE in roughly 6 months at which point I will see him in follow up."
