Palpitations and spikes in BP

I get those palpitations that could be PVCs or PACs, I've had them for years but they are more noticeable these days. 3 Nights ago I had a completely sleepless night with them and my blood pressure was very high, in the red zone on my monitor (I should also say I was very anxious at the time). In hindsight, I should perhaps have gone to Urgencia but it has all settled back to my version of normal. My BP is normally somewhere around 120/70 sometimes a bit higher sometimes a bit lower. I do have occasional spikes and when I decided to go to the doc this morning to chat about my recent experiences, he took my BP and it was 160/100 so he has put me on a tablet called Acetensil 20mg, 1 per day (I live in Spain, I've just Googled and the ingredient is enalapril maleate) . I did explain that the spikes in a doctor's office were normal for me - once at Urgencia several years ago, I went into a hypertensive crisis and they wouldn't let me go home for hours, until my BP came down. Reading about this medication, 20mg seems a high dose to start off and I am worried it will crash my blood pressure, if I take it when I have normal readings ..... any advice welcome, I am due to start taking them in the morning. Oh and I am 73 years old, slim, don't smoke, walk for at least half an hour a day and take 50mcg of a thyroid med, levothyroxine, no other meds.
