Once filled in you should qualify for Disability Living Allowance (DLA / PIP), Attendance Allowance (AA) or Incapacity Benefit (IB) under what are called 'special rules'. From the DWP. This is not means tested so you income has no effect on the amount you are entitled to.

had emphysema now 16 years and never been offered lung transplant
I have emphysema too and my GP told me tha a lung transplant was not an option?? Don't know why, he didn't say.
I was referred on 23rd April for accessment for lung transplant at newcastle.
