Medicines do not ever stop working but sometimes the illness or condition worsens so they appear to.

Not really true for opioids where the body gets accustomed and dosage has to increase to get same pain killing effects.

True I should have said these heart medicines do not etc. Opiods have receptors in the body which when excited multiply needing more drug etc etc.

Drugs like Flecainide, beta blockers or calcium chanel blockers have no such receptors so are not affected in this way.

I read somewhere that when you are put on a beta blocker the body makes more receptors for what the beta blocker is blocking. The problem comes when you come off it too quickly and these extra receptors are soaking up the adrenaline that is no longer blocked giving you the jittery withdrawal symptoms. It seems to make sense . Lots of people here have found coming off beta blockers difficult.

I have read that some people can become tolerant of certain medicines and that they lose their effect. Sleeping tablets come to mind and I've heard people say the same about arthritis tablets. But heart tablets are, I'm sure, different in the way they work and will keep working properly. Sometimes the condition they are being given for can deteriorate or improve, in which case a different dose, or a different tablet, is needed.
