Trigeminy

Has anyone on here been told they have trigeminy. I have a Kardia and getting almost all unclassified results. I sent a couple to be analysed and they said:

Multiple ventricular ectopics and trigeminy.

I am having some AF after 7years free after ablation.

Any thoughts?

I have bi and trigeminy, ectopics and svt 🤦🏻‍♀️ I have a pacemaker and take Bisoprolol.

Hi. Was your beat fast and us this why you take bisoprolol. Did the doctors say if trigeminy causes damage to the heart? Thanks for reply.

bantam12 profile image
bantam12 in reply to Dodie117

Yes I have a fast rate so the Bisoprolol mostly controls that and because I have bradycardia as well the pacemaker sorts that 🙂

Hi why would you need a pacemaker if I can ask? I am similar to you with multiple ectopics and bigeminy and SVT. Cannot take B Blockers as resting heart rate around 50bpm. Failed ablation and heading for second ablation.

I have the pacemaker for bradycardia and pauses caused by heart block, I was offered an ablation for Afib but I refused it, failure rate and lengthy recovery time wasn't an option for me.

Ventricular or atrial trigeminy? Can you post a photo?

Thanks for reply - sorry so late getting back to you.

RESULTS

RATE: NORMAL

The 'normal' resting heart rate sits in a range of 60 - 99 beats per minute. Your heart rate is within normal limits.

SINUS RHYTHM

Your heart is beating normally and is in Sinus rhythm with a rate of between 60 and 99 beats per minute.

MULTIPLE VENTRICULAR ECTOPICS

We have observed ventricular ectopics (VEs). These are extra beats originating in the bottom part of the heart. This is extremely common and can be a normal finding in healthy people.

VENTRICULAR TRIGEMINY

You have Trigeminy which refers to two normal beats that are followed by an extra beat. This may be considered outside of normal limits. If you have symptoms of any sort that concern you you should consult a medical professional.

SINGLE MORPHOLOGY (SAME SHAPE)

Your Ventricular Ectopics come from a single site in the lower part of the heart.

NO ATRIAL ECTOPICS OBSERVED

A atrial ectopic (AE) is an extra beat originating in the top part of the heart. You have none which is completely normal.

NORMAL QRS PATTERN

The QRS morphology indicates how the bottom part of the heart (ventricles) contract. You are within normal limits

FULL DISCLOSURE

Measured rate: 69bpm

SYMPTOMS AT TIME OF RECORDING

Wow, such an amazingly detailed report, and such a sharp ECG image when you blow it up. The report seems very favourable. Are you particularly sensitive to the ectopics? A 72 hour holter might show whether the VEs are linked to your AF. Not sure why they would be. Atrial trigeminy then yes.

Thanks. May I ask if you are a doctor. I notice you are new here and asking what seem like very informed questions??The ECG is from my Kardia and the report from Kardia cardiologists - a service available from them for £5 which is useful if waiting to see doctor. I am getting odd bouts of af also.

My sympathies lallym - I found PVC's in such frequency very uncomfortable. It could be that something has irritated your heart and is causing irregularities. Can you tie the start of it to anything specific such as a major change of diet, activity or drugs?

I had horrible runs of tri- and bigeminy when I tried to reduce my Flecainide dose some years ago. It got so bad that my cardiologist advised me to go back to the higher dose and the ectopics settled, never to return in such a concentrated form.

The most unwelcome thought is that some of your ablated pathways are reconnecting . . . hope not. Can you send your Kardia trace to your EP?

Thanks for reply. Nothing new but have gut issues. However, they have been around for years so although I always felt they contributed to my AF I have been keeping some control with diet, probiotics etc. I am now in Ireland and the EP who did my ablation is in London (I was resident there until recently) so having to start again with doctors, referrals etc. 😢😢 Putting it off due to Covid. Appreciate input.

Sorry this is a nuisance. Have you talked to your doctor about beta blockers? Take care

Hi Rosy. Thanks for reply. I have not had much contact with Doctors here in Ireland yet. - moved here last year. The beat is not particularly fast so not sure about bisoprolol if I can avoid it. I will have the conversation. Expecting vaccination in a couple of weeks so will be more confident about going to doctors and hospitals then.

Hi, I had episodes of bi and Trigeminy, all lasting about 7-8 hrs during the night for about 4yrs. They all came on about midnight-2am after I had had a late heavy meal and would wake me up with the very repetitive thumping and pause, usually accompanied with heartburn. I managed to stop the episodes each time with exercise in the mornings which sort of threw me back to a normal sinus rhythm. My rate was usually about 60 bmp. Unfortunately I then changed to PAF December 2019 and here I am. I now know that I have vagal (rest and digest) induced PAF wish I had known that back in 2015 🙁 Chris

Yes, I am convinced this is related to gut issues. Sorry to hear you are have PAF back. I too am getting the odd bout after 7 years of peace. Thank you for your input.

I wasn't told I had bigeminy, but identified it myself from my Kardia traces. I went through a few months of getting it nearly every day after eating. It sometimes went on into AF as well.

Yeah, gut problems. My digestion has been very much better since I've started taking vitamin D and vitamin C supplements, and the bigeminy has stopped. So maybe diet has an influence? Good luck with it, and I hope you get this resolved.

Thanks. I do take vitD but not C. Plus magnesium. Must add fit C!

momist profile image
momist in reply to Dodie117

I only started taking Vit C recently, mainly because I saw the recent video from York Cardiology. (Sanjay Gupta). It seems that there 'might be' a beneficial effect over the incidence of AF, and since it is well tolerated with little side effects and perfectly safe it seemed daft not to! I now take a capsule of 1000mg per day.

I've had trigeminy and bigeminy in long runs of several hours on a number of occasions. They seem to relate to me have overdone things physically (often does not seem to take a lot of overdoing it). I think lifting heavyish things can also result in these incidents and sometimes a glass of red wine will do it. They are very unpleasant and I started getting them following my 2nd ablation which has so far been 'successful' re stopping Rapid Paroxysmal Atrial Fibrillation attacks. I get fewer of these 'attacks' now as I think I have learned to really slow down and cut back on my physical effort which isn't good as I'm pretty overweight and unfit now. I use Bisoprolol 1.25mg (which I normally cant get away with due to side effects)as a pill in pocket and if that doesn't work I can use flecainide as Pill in pocket as well. I also get tachycardia frequently either following too much exercise (very little in reality) or several hours after doing too much. I think this tachycardia can often turn into the trigeminy and bigeminy attacks. I haven't had RAPF since the 2nd ablation in 2018 so it doesnt appear that the trigeminy and bigeminy is a marker of imminent return to PAF.

Mine seem to be a lot more frequent. I’m not finding them too disturbing but they do interfere with my sleep. I can’t relate to any of the triggers you outline because seems to be there almost all the time. Thanks for your input. Very useful.

There are five strips there beginning at 2:37:03 a.m. and ending at 2 :37:43 for a total of 40 seconds. There is a "1" preceding the time. Do I interpret that to mean that you used a one lead Kardia?
