You cycling is most impressive. Does it send you into afib? If in NSR what does your heart rate rise to? If it is not causing complications exercise is most helpful in maintaing and improving cardiac function. By IBS do you mean irritable bowel syndrome. Commen meds for this are antiparasympathetics which would be relatively contraindicated with afib and cardiomyopathy. A gastroenterologist would be more knowledgeable about other and more effective treatments. Do you know your ejection fraction.? Ask the cardiologist what are the specific indications for a pacemaker in your case.They certainly can be complications resulting from the implantation and the device itself. Ask whether the proposed pacemaker contains a defibrillator. Best of luck
