Uh... Oh... New Phase Awaits?

Dx 12/09, G4+3

Surgery 01/10 – Failed

SRT 2nd quarter 2011 – Failed.

Avodart worked for awhile. Eventually HT (Lupron + Casodex)

in mid 2013. Worked great.

Since then lengthy HT vacations

12/19/22-6/22/23 ---Orgovyx. Worked well so was able to stop...BUT NOW

Starting to look like castrate resistance:

9/14/23 PSA = 0.26 Total T = 41

11/28/23 PSA = 1.2 Total T = 51

1/15/24 PSA = 1.7 Total T = 64

3/18/24 PSA = 2.6 Total T = 56

We are going to do 1 more in 2 months. Then ?

Am I castrate resistant?

Also, I have been off Orgo since June, 2023.

Is this T decline to be expected? I'll be 78 in June.

You are castrate resistant when the PSA value rises while you are on hormone therapy. As you stopped Orgovyx a rise in the PSA value can be expected. The testosterone level recovers quickly after Orgovyx.

Thanks for replying. But, as per my post, I have been off Orgo since June. My T-level is very low.

Probably testosterone does not recover from the Lupron treatments. If you start your ADT with Orgovyx the testosterone recovery is relatively quick.

The Lupron/Casodex HT was in about 2012. I did recover from that.

As I said:

T is bad as delineated in my OP. I think it might be the aging process coupled with an extremely sedentary lifestyle due to a spinal injury.

IDK why you imagine you are CR. Your T is not at castrate levels.

Here's a plot comparing Castrate-Resistant Prostate Cancer survival %S versus different levels of testosterone nadir. There is a difference between > 50 ng/dl and 20-50, and < 20 ng/dl.

You can draw your own conclusions...

Thanks for all the responses . . . and the information/education. I interpreted CR as rising PSA in the setting of lowering T. But I missed the castrate part. My T is not quite that low.
