Any input on how long it takes for PSA to start dropping if Pluvitco is going to work?

For me by the second infusion. It just did not turn out to be durable for me. Lasted 6 months. PSA started going up.

After 3 to 4 weeks. This article has info about temporal changes of the PSA during Lu 177 PSMA treatment and outcome.

My husbands started to drop 4 weeks after the first infusion and has now dropped significantly. Unfortunately there has been significant spread of the tumours throughout his skeleton so the treatment has been stopped after the 3rd infusion despite the PSA continuing to fall.

Do you know what's the cause of the spread? And why would Pluvicto be stopped if PSA is dropping?

They don’t believe it’s working and don’t understand why the PSA has dropped. We are waiting for the bone scan results and the next PSA test in 2 weeks then we are going to discuss further as my husband wants to continue. That said he is getting more pain flares this time.

How did they find the new tumors? PSMA Scans? Or bone scan?

They do a CT scan after every infusion and this time there was significant spread. He has a bone scan next week,

I have had 4 infusions so far. Blood test three weeks after first infusion PSA started to drop and continued to drop. Before first infusion PSA was 68.37 three weeks after fourth infusion PSA was 9. Will have PSMA-PET today, next week blood test and 5th infusion.

Russ, thanks for your reply. As you know we all respond differently and at different intervals to Pluvicto, as Seebs9 indicated. Feel free to inquire further. stay strong. All the best to you.

In my case it took just before my 3rd treatment to drop from 62 to 34 Oncologist super happy, said they are seeing a 'delayed response' in some people. Hang in there Russ.

Thanks. If I get a 50% drop, I'll still be in the 190's. Hoping for the best.

Here is a great YouTube video about response by Dr. Kwon, with Mayo. Go to about 26 minutes. youtube.com/watch?v=bwUJJP6...

Thank you for these. I watch both today. My psa is in the 300 to 400 range so I don't look forward to miracles, but it's nice to know what to expect.

Pluvicto failed for me from the beginning. I had a PSA of 1.56 at the beginning of treatment and after the 3rd my PSA reading was 10.18 I stopped the treatment after that. Now, that doesn't mean it won't work for anybody else, chances are it will. It just didn't for me. I have exhausted all other treatments. I'm going on 10 years with this disease now. I have to rely on clinical trials now when available. Until then, I'm back on Jevtana which kept my PSA between 1-2.

Thanks. I'm sorry it didn't work for you. My PSA is 435. I hope it will go down some.
