Are you metastatic and castration resistant? If so, Zometa or Xgeva may be for you.

If a dexa scan shows a T value of -2.5 or more you are osteoporotic and you should be treated with denosumab or zoledronic acid.

If the cancer is castration resistant with bone metastases regardless of dexa scan values treatment with denosumab or zoledronic acid may reduce the incidence of bone events such as fractures, bone pain etc.

Estrogen preserves bone density in both men and women. Men normally convert testosterone to estrogen, but since ADT lowers the first, it also lower the latter. So adding some back, via low-dose patches, gels or creams, may help slow bone degradation (for both men and women) when an older person is not producing adequate amounts of estrogen.

I do not know if it can help to significantly reverse the loss of bone density that has already occurred.

Weight-bearing exercise also helps to preserve and strengthen bone.

According to my Endocrinologist, we can't really rebuild bone that has been lost to PCa, but we can strengthen the remaining bone.

Zometa and Xgeva will help. Need more info on your condition. Are you on ADT? How much do you exercise and what kind?

I will rather prefer Alendronate 70 mg once a week tablet if T score on DEXA scan is still below -2.5. If it is above -2.5 then you are truly in Osteoporosis phase (not osteopenia)in that case Zometa or Xgeva infusion may be needed.

By the way besides resistant exercises, turmeric, Vit D and high calcium foods help too.

I like to use Rubber bands (available on Amazon) . By stretching them, I get sufficient resistance exercises.

Before resorting to Zometa or Xgeva, get your estradiol [E2] tested. If it is <12 pg/mL, that probably explains the bone loss. Use low-dose E2 patches to restore the basic level of E2 needed for bone health.

Look for a bone health supplement that contains vitamin K2, magnesium, zinc, boron & a moderate level of calcium.

I have been on xgeva for four years. All those years on ADT drugs takes it's toll.
