I had this treatment in 2016 at the Technical University Munich. One treatment made all the lymph nodes mets PSMA negative and they have remained PSMA negative so far.
They do not treat what they can not see in a PSMA PET/CT. Some places, like in India they do a treatment without mets in a PSMA PET/CT but it may be risky since a lot of radiation could go to kidneys, gut , salivary glands in absence of metastases 4 to 5 mm or larger.
The treatment is very well tolerate some fatigue, transient dry mouth etc. People with large number of bone mets or diffuse infiltration of the bone marrow could have more side effects because of suppression of the bone marrow by the radiation.
If your PSMA scan had high values of uptake in cancer sites, then Lu-PSMA treatments may be of significant benefit. You can get them overseas now. And probably next year in the USA for mCRPC. So the PSMA on PC cancer cells can be useful both for imaging/diagnostics and for therapy with radio-ligands. That is why it is sometimes referred to as Theranostics.
Great... it is a bit unclear terrain...Would be very informative to follow what is Theranostics....
It is what we need....not only scanning but curing thru radioactive drugs...
Hopefully less toxic and reaching the real target without any scorched earth killing...
