Hello! What do you know about Lutetium-177 PSMA-617 (LuPSMA) therapy? Has anyone tried it? What were the results and how did you feel afterwards? Thanks

I have recently finished the "Vision" trial which used Lu-177..It worked very well for me, taking my PSA from 2300 to 135. Unfortunately the effect was somewhat short-lived...The side-effects were mild but cumulative, I had 6 treatments and by the 6th one, I had had enough..I now have permanent dry-mouth and have lost my sense of taste and smell..

I had 4 infusions of Lu177 from Nov 2018 to May 2019, Psa fell from 25 to about 1.6,

But I also was on Xtandi after 3rd infusion and maybe that made Psa fell to 0.32, at 1 year after start of Lu177. The followup PsMa scan in August 2019 had a report saying no soft tissue mets could be seen and bone mets were healing up.

No doctor said I had any mets in scan that had not responded to Lu177.

But Xtandi has probably stopped working and now Psa is back up 11. So the usd $28,000 bought me about 18 months suppression of Psa, but my last and 7th PsMa scan was only last week and I won't know what they found until next Thursday and I see my onco again.

I had countless bone mets, some the size of a pea, but Lu177 can only do what it does for a short time in body due to its short half life. No man would want Lu177 to have a longer half life or bigger doses or else side effects could be a real problem. Mine were not, but maybe I must have more Lu177 because some small mets may have grown large, or some that were reduced in size were not fully killed by 4 infusions, and have grown big again.

Docs here said I had a good response with Lu177, and the inventors in Germany of this targeted nuclide treatment said the median time of extension to life is 14 months where PsMa scans indicate a benefit is possible. I've just had 14 months.

I am cycling 200km a week+ and have no symptoms of Pca, and no side effects of Lu177.

I do have lingering side effects of chemo that failed in late 2018.

ADT + Cosadex and then Zytiga failed before I began chemo.

Research at PeterMac in Melbourne is continuing to find out why Lu177 does not work better, or even at all for some men.

But for me, I would be in palliative care now if I didn't travel 300km to Sydney for Lu177 last year from my home city of Canberra.

I've written a pile more here on my experience with many treatments since diagnosis in 2009 at age 62 with a Gleason 9, Psa 6, inoperable. I've survived 10 years+

Thanks, Patrick Turner. My husband is 57 years old. He was diagnosed in November 2018 with stage IV prostate cancer and underwent a radical prostatectomy in January 2019. He's been on Eligard ever since. The PSA before the operation was 11, after the operation it decreased to 0.17 and then it started to increase until it reached 33. Our doctor has not added any other treatment besides Eligard so far. Now his leg is starting to hurt, and we continue exclusively with Eligard. I desperately need help ... (and I hope google translate did a good job above!)

Lu177/ac 225 failed for me. I had 3 doses. My lymph node mets responded well, but not my bone mets which are now extensive and symptomatic. I was very disappointed as my Psma scan prior to treatment showed that I was intensely Psma avid.

Have you considered Ra223, aka Zofigo?

Lu177 with small amount of added Ac225 is supposed to work well with men with high PsMa expression, but I found I needed 2 infusions of Lu177 before much action on any bone mets was seen in scans, but soft tissue mets in lymph nodes took a dive.

I needed the whole 4 shots of Lu177. Followup scan showed big reduction of all mets and some bone healing. But Pca seems to always grow back again like a weed. I know men in a trial of LU177 for 30 men in 2016 who each got 4 shots of Lu177 and 70% did OK, but one had 7 shots, with the 3 shots outside the trial. He had much worse bone mets than mine, but he's still alive and well.

Research at PeterMac hospital where the trial in 2016 went on is continuing on why some men don't get a response they hoped for.
