Okay, Bob, long story -

This is all about bone tumors. Frankly, I think it saved my life even though it was nominally for “pain” and not for tumor suppression (I am convinced they thought I was dead without nuking my spinal tumors, though).

They targeted both my C2 and T4 tumors as these presented the most concern - they were large and involved big portions of the vertebral bodies. It was was both horribly tough and laughably easy - the personalized plastic cage they use to align you for multiple treatment is a claustrophobic nightmare, but the actual treatments were only seconds long. The early days were easy, but as the treatment continued and the tumor (and, yes, living bone and tissue, no matter how well they collate the beams) died, the pain was horrible, particularly in my neck, and also my throat where I received some radiation burning. My wife hated it.

In sum, the collateral damage is a reality, because radiation kills and bone doesn’t come back from that insult. Killing the tumor also meant that my C2 vertebral body slowly collapsed into itself. C3 was also affected. It may mean substantial stabilizing surgery someday (the docs wanted it yesterday, but I was lucky enough to come out with any neuropathy despite the permanent “pathological fracture” in my neck, so I wait...)

External beam is a tough call, Bob. I was newly diagnosed, starting chemo, and under pressure to throw the kitchen sink at my cancer. I was desperate.

It worked out for me, as I have largely healed (I think Xgeva helped and I forced myself into physical rehab) and I know the tumors threatening my spine are well and truly dead. But I also have tumors in my pelvis and left femur that were untreated by radiation. Radiation could leave these structures severely and permanently impaired, and if Xgeva has become toxic to me, there may be no true healing or therapy to overcome this.

So.... I think EBR works, but like all of our available treatments, it is making a deal with the devil. - Joe
