Trying to get to retirement before HT
Hi All, I'm currently 65 years old. My PSA level before the biopsy was 8.1, and my Gleason Score was 4+4. Fortunately, there are no signs of metastases. Although my treatment hasn't started yet, it's likely that I'll begin radiation therapy in early December. I'm currently overseas and won't return

Is the a standard/preferred course of treatment for mixed ductal PC (post- RALP, post recurrence in prostate bed, post radiation)?
I’m also a tiny bit concerned about the psa reading.. was expecting <0.2… I know 0.4 is not huge, but after 3 months of
ADT
and a full course of radiation, I would have thought undetectable was likely. Thanks for any info!

TET Testosterone Treatment on Clinical Medicine
If true this makes the need to take TET to zero during
ADT
treatment not only needless, but harmful for men. Its a bold premise that goes back to Dr Huggin's original 1941 study on which most SOC for TET supplementation is based.

In 2019, it became stage 4 and radiation/chemo/
ADT
. Thanks be to God, my PSA is still undetectable though I have a few bone mets (rib, spine). We pray for God's Mercy on all the dying, Christian, Non-Christian, and those with no faith.

BAT Question
I would like to know for those folks experimenting with BAT or actively doing so with their MO once your T rises again and then when you reinitiate
ADT
do you go through the whole hot flash cycle again? Is it the constant back and forth feeling of feeling great and then like crap?

8 Month Follow up Report from Lu-J591
In 2014 my PSA was over 5.0 and went on bicalutamide + dutasteride as alternative
ADT
. Positive PLNs on PSMA in 2019 led to hemipelvic SRT and 6 months
ADT
. However, PSA rising in 2021 to 0.20 and repeat PSMA PET showed recurrence in two new LN sites, one para-aortic, so now metastatic.

The term 'Castration' resistant/sensitive is offensive
Why isn't it called '
ADT
' resistant/sensitive? It's bad enough with all the side effects we go through being on
ADT
, but then to be labeled with the term 'Castration' when I still have my testes and generate T when not on
ADT
, it's offensive to me. Just something to think about...

Update on "Zytiga failed, Xtandi next" from 8 days ago
My "winter" MO (Fl vs Mass) says the active hip lesion means the Zytiga is failing, at least at that one area only but recommends Xtandi and I am now hormone resistant after 19 months on
ADT
plus Zytiga/prednisone. I asked my "summer" MO the following.

Looking for suggestion of clinical trials for mCRPC
I have had SBRT (2007), Chemotherapy (2010/2011),
ADT
using Casodex/Lupron/Zytiga (2012-Current). Interested in joining trials that involve PARP inihibitors, immunotherapy, or other new/novel therapies. Can any of you suggest trials currently accepting candidates.

PSA after 4-5 months on Lupron (Scholz)?

What is role of hormonal therapy drug when taken with targeted therapy, eg Piqray or Ibrance?
I’ve been curious about this and wondering if anyone can explain. I want to better understand the role of the hormone therapy eg Faslodex when taken with targeted therapy such as Ibrance. Does it just reduce the amount of estrogen available to the cell and “lightens the load” on the targeted therapy?

Aspirin trials
I have recently finished hormone therapy, last 3 monthly injection April 2023. Good result so far with PSA at <0.01. Met with my Oncologist today and he said there were trials using aspirin as a potential drug to reduce potential recurrence. Has anyone come across this and, even though the results
