After the Xtandi failed it was back to chemo, this time
docetaxel
. I also started some targeted high dose radiation on some bone mets to beat it back and help with bone pain. I did about 6-7 cycles of the
Docetaxel
then it failed and PSA started up again. So now it’s about 3 years later 2019.

good news
Blood work results prior to my 8th
docetaxel
injection this week were very good , PSA dropped again from 5.54 to 3.48 , shortness of breath is increasing so a check of the lungs is in order and a check of the old ticker , but feeling pretty good all things considered......

Starting Chemo - Thoughts How To Manage Side Effects
I am starting
Docetaxel
on Tuesday. My Lupron plus Zytiga failed immediately. MO says this is the next best step. I so appreciate all of you sharing your personal experiences with all of us when we have questions. Please let me know any of your thoughts if you have time. Questions are: 1.

Why continue LUPRON when ADT not working?
First therapy:
Docetaxel
with Lupron, then just Lupron for 1 year, 4months. Then added Abiraterone for 8 months. Currently stopping Abiraterone and I am having EBRT on femur. In three weeks I begin Radium 223.

Docetaxel not working
I have had 2
docetaxel
infusions so far with 3rd infusion scheduled this Thursday. Psa has increased from 120 to 174 and alk phos has increased from 590 to 617 since last infusion. Should I even go for 3rd infusion since
docetaxel
seems to be ineffective ?

Mum - second-line treatment
During the meeting it was agreed that mum would go on to
docetaxel
. I asked him about adding in nintedanib and, although he isn't convinced it will add much benefit, he has agreed to mum having it.

Castrate resistant - new spot?
My dad is on Zoladex, he had had radiotherapy to the prostate and 4 x
Docetaxel
to treat his aPC last year. It should have been 6 x
Docetaxel
but it was cut short due to the Covid scare. With this, his PSA dropped to 1.1 (originally 44) in October, but January's PSA test bumped up to 7.2.
My dad is on Zoladex, he had had radiotherapy to the prostate and 4 x

Need suggestion PSA 878 after one month of 3rd Chemo
The initial plan was
Docetaxel
50mg/m2 in 100ml NS over 1hr -- 75mg over 2weekly. However, due to low platelet count and HB count, his chemo was pushed by 1 or 2 weeks. we had 3rd Chemo on 13 Jan 2021.

Consultant Meeting with Mum
He then talked about a chemo drug called
Docetaxel
. I asked him about Nintedanib combined with
Docetaxel
and he said that he didn't think it would add any benefits for mum. I just don't understand why he is so pessimistic about it helping.

Xtandi appears to be ineffective
I hear Cabazetaxel has a little less side effects than
Docetaxel
, what is your experiences out there? my Dad tolerated it well but no having taste made him so depressed. He loves to cook and loves to taste his food. PSA is up to 160.

Bone Marrow Failure After 2 Rounds of Ra223
After 9 rounds of
Docetaxel
(the last 9/2/20 and it really seemed to work) we started Ra223 on 11/20/20 with round 2 on 12/18/20. My blood counts tanked with platelets at 10 on 2/8/21.

Another stable guy from MA
The 10
Docetaxel
rounds over 7 months was a slog, but my guy's had stability for the 7 months plus 8 months post-chemo--15 months total. He does have manageable, but annoying, neuropathy from all that chemo even with cold mitts and socks.

What is considered Castrate resistant?
Originally his psa was 1400's, did 6 rounds
docetaxel
, lupron for about a yr. Which brought his psa down .02. Recently they added Xtandi because his psa was rising every month up to almost 4. Does this mean he is castrate resistant?

Diagnosed stage IV aged 50, still going 8 years later :)
PSA started rising quickly again, so after a PSMA PET scan in 2015, radiated a new tumour on spine, unfortunately that didn’t seem to help, so followed up with ADT in 2016 and
docetaxel
in 2017. PSA plummeted to 0.01 and stayed there for a year, so stopped again.

Ra223 Xofigo and Cabazataxel compared
Since diagnosis 2009 at 62yo, with inoperable Gleason 9, but with low Psa only 6, I had failed RP attempt in 2010, but has EBRT + 2 years ADT, then pause showing Pca progress so I am still on ADT, and had salvation IMRT to PG, Cosadex, Zytiga, 5 doses

, all not stopping Pca progress hence move

Three clinical trial options after Zytiga fails
He’s 71, dx’ed nearly 3 years ago, and has already utilized Zoladex,
docetaxel
, and Zytiga and some targeted radiation. He’s had widespread mets since dx but largely feels pretty well (aside from Covid knocking him on his rear in October).

Are Anti-oxidants to be avoided with aCRPC?
I was getting vitamin C infusions to help with my immunity prior to my
docetaxel
infusions and have continued to get them the in-between weeks. TA mentioned anti-oxidants counter the ROS needed for cancer cell death.

When to do 'advanced' tests?
He has not yet had
docetaxel
(cancelled last year due to Covid) so I suspect that might be the next step at some point? Would any of these tests be of any utility in looking at that treatment option – or are they irrelevant to that decision?
