What is the best treatment for a butterfly rash that doesn't respond to antimalarials, steroid cream or pro topic?

Have you tried 'icing' the area? The blood vessels are what makes it warm and red. If you take an ice cube in a napkin or tissue, it cools and reduces color. Not a cure obvi, just a simple way of making it less obvious if going somewhere special,
and it feels somewhat better.
Also wear sunscreen 'always', rain or shine... photosensitivity is the trigger along with the illness itself.

Thanks for your reply Marcie! I wear factor 50 every day, which I get by prescription. Obviously it doesn't help with the damage that's already been done (lots of broken blood vessels on the right side of my face), but hopefully it will slow its progression as its beginning to appear on my nose and the left side too. Icing it won't work as the cold makes it much more noticeable, unfortunately I'll be attending a clinical meeting in dermatology next month, so hopefully they'll be able to suggest something that will actually work this time!
