It's not instead of Gleason, it uses Gleason and the other risk factors (PSA and T stage) to come up with a prognostic risk stratification scheme. There are several alternates to the NCCN scheme. I think that many such schemes are defective in that they look for mortality instead of biochemical recurrence. I also think predictive schemes are more useful to patients than prognostic schemes.
