I read this article in the UK's Telegraph newspaper today and wept.

The NHS Blood and Transplant (NHSBT) authority now think that 177 organ donations every year are stopped by family members because they are uncertain that it is what their loved ones wanted. According to NHSBT that equates to 450 potential organ transplants (most people donate more than one organ). That's 450 lives lost because people did not have a conversation. And you can't blame the family members.
If you are asked a direct question and you don't know the answer what else can you do but say no and maintain the status quo.
