Dear Charmaine I work with young adults (16-19) and in my role recently I've realised that many still require support from their parents to clean themselves after a bowel movement. This means that they are holding on all day whilst at school or when away from their parents.

Is there any age appropriate training or support I could provide to help these young people become more independent?

Some students have weak grip and dexterity issues so I wondered are there any exercises or physio that could help?

Hello SPSM

Thankyou for your enquiry.

I would recommend supporting your young people through “two” approaches;

1. Bowels;

2. Training

1. Bowels

I would recommend supporting your young people with the promotion of Healthy poo and Healthy wee. Reviewing routines and promoting 6-8 cups of clear fluids each day, across the whole day, regulating meals snacks, promoting correct positioning upon the toilet, promoting a toilet routine,

Review bowel motion in accordance to the Bristol Stool Chart

By doing this you can support your young people in regulating there bowel movements. Ensuring results are soft, reducing the ability to hold on.

Should constipation be highlighted, then your young people can be supported with this.

I have produced a series of videos, offering advice and support for families, carers and professionals. I’ll attach one of my videos, focussing upon “poo” for further information and support for you;

2. Training.

With regards to training and how to support your young people with wiping, there are several ways that you can support with this.

For many of the young people I support, I introduce a social story, explaining how to wipe, with visual examples. The social story is introduced as part of a toilet routine. For example for many it is a story, read or viewed while on the toilet or for others at set times each day to enable the concept to become familiar.

Bobby can use the toilet, book can be used as a visual social story as it clarify, how and when to implement this skill.

With regards to learning the skill of wiping there are several approaches.

You can support through either introducing toilet wipes and toilet paper. Promoting the skill of pulling a toilet wipe from the pack and practising wiping chocolate spread off of a prepared covered orange, away from the bathroom. Through this approach your young people can begin to develop the fine motor skills of clasping paper and the concept of wiping.

With regards to learning the skill of reaching around the body.

You can support through introducing a game of “who can reach the paper”. Through a dedicated timed activity; A small amount of toilet paper can be celotaped to the bottom, clothing. The game can be individualised and prizes can be won to promote motivation of wanting to reach the paper.

These approaches are Starting approaches in your young people learning the skills of Passing soft poos regularly and being able to wipe.

Through my role I provide individual support through direct solution assessments as well as providing training for charities, organisations and professionals. If, this is something you and your organisation would like further support with, it is possible for a training pack to be formulated just for your area, to ensure it addresses all the areas of needs you have identified.

If you would like any further information or support, please feel free to contact me
