This questionnaire is about your Quality of Life whilst living with acute leukaemia. We would like to ask you about your experiences of living with the disease – throughout your treatment journey and beyond.
The survey results will be used to help understand the key issues, experiences, and unmet needs for patients and caregivers throughout the journey with acute leukaemia.
Taking part in this survey is voluntary.
Published reports will not contain any personal details.
Who should complete the questionnaire?
The questions should be answered by the person who has been diagnosed with acute leukaemia. If help is given to complete the questionnaire, the answers should be given from the point of view of the person diagnosed with acute leukaemia – not the point of view of the person who is helping.
