You and I seem at a similar transition regarding efficacy of PD meds. I'm pigd beginning year 7 since my dx as PD. Citicoline and cholinergic herbs are helpful for me - along with 25/100, some mucuna, amantadine, plus nortriptyline, ropinerole, and melatonin for overnight.
My husband with Parkinson's doesn't find his Sinemet to be very effective (diagnosed 12.5 years) especially as he has a very strange gait on which none of the health professionals he's seen recently seem to want to comment.....apart from increasing his meds! Tbh it doesn't seem to matter whether he takes his meds or he doesn't, so what's the point? His mobility is getting worse so that when we travel, we have to take a wheelchair but at home, he manages to go up and down stairs without a problem (meds or not)......it's a mystery. However, after reading certain articles on this site about Cholinergic Dysfunction in PD and other posts elsewhere about the death of cholinergic neurons in the PNN believed to be the cause of some PD symptoms i.e. attention, walking & posture; I think my husband should try Citicholine - aspergerian which product do you take and could you refer me to the cholinergic herbs you have found helpful?

I am neither an MD nor a PhD. I shall summarize what is working for me in recent weeks.
Some of the medical literature regarding falls calls attention to cholinergic dysfunction in the PPN (as you mentioned). Citicoline has been found helpful for PwP still taking traditional PD meds. The PPN/PD literature has been increasing since 2007, thus findings are relatively new. As I enter what seems to be Stage 3 of my PD (with balance impacted, falls, mild tremor occasionally...), I was researching falls when I chanced upon the PPN studies, many of which mentioned acetylcholine and cholinergic problematics.
i went to iHerb and found a selection of citicoline supplements. I selected the Healthy Origins brand (based upon price). I've been trying several herbs with cholinergic effects reported in one or more Pubmed studies. Rosemary seems the most potent (capsules), ashwaganda next, then bacopa.
The herbs and citicoline have helped, augmented my traditional PD meds. When the whole combination gells in my body, I have energy, much movement is restored. However, the pharmacokinetics of the aforementioned herbs and traditional meds seem somewhat to differ, so I'm still working out a dependable protocol, even if temporary. Off-time can still happen, and I've learned to set up the next several doses while the most recent dose is still providing On-time.
Cholinergics aren't for everyone with PD. Be careful.

Good luck. As I restore On-time, I am making sure I include more exercise in accord with J Pepper's insights and related exercise-PD studies.
Ah, if only Glen could do fast walking (or any walking) - he tried several years ago after meeting John Pepper - but he has arthritis, two complete knee joint replacements which he's struggling with, a bad back and the unusual gait, all of which now make it virtually impossible for him to either stand, or walk; although as I mentioned previously, he can walk up and down stairs ok
Yes I have wobbly legs too. Only have tremor in left arm...but feel like legs are now starting to be affected. I'm not on meds yet. If I excercise it is better
You appear to be taking too much medication, but I am not a doctor and cannot comment on that. It sounds as if you are very physically weak! You should try to do some meaningful walking, but go about it carefully. Look at my profile and contact me.
have you seen a doctor about that..you reall need to do something about that as you will get worse,,first get your self walking in the morning i do it 7 days a week 52 week a year, push ups sit ups go into a gym have a look at what you can do.or even join a gym.like i said before im 71 and have abs and i dont go to a gym.if i can do it so can you buddy.regards john,
When my legs got weaker and felt shaky, a Rock Steady Boxing coach suggested getting ankle weights and doing leg lifts from a sitting position. Too soon to see a change, but I'll keep you posted.
