Hello,

I had a wonderful reply on my question FCD type 2b or HIE mature ischemic insult.

I read the reply today now cannot seem to find it?

The information was very useful to me and I would love the person from the site who answered my qn to correspond with me again.

I almost wonder have I dreamed of the reply 😂..I cant find it anywhere!

The main question I had is if they are two separate conditions both looking differential on MRI, why would a Neuro/Radiologist have concluded on my sons MRI that

It "may present areas of FCD type 2b or areas of mature ischemic injury"

Its a very wishy washy conclusion.

Surely they present differently on MRI?

Is it to do with it being a child brain ie small and hard to define what is the condition they see in the frontal lobe?

Dr_J_Choi I have stupidly deleted your reply which was on my question about the site being redundant, I wanted to wipe that question seeing as you replied thank you, yet Ive deleted your reply too 😓

On a 4 year olds MRI, does HIE mature ischemic injury present differently than the FCD type 2b balloning neurons/ dysplastic neurons?..I find the MRI report qurstionable, it concludes to us in a way "may be areas representing....or...."

How can they not give us a difinitive conclusion...its either a or b...surely they look completely different on inspection and on frequency etc?

Ive read up that FCD type 2b can be congenetal or environmental insult yet also read there is no clear cut cause for it either...

I have also read that its clinical symptoms and severity depend on where found in the brain.

Our sons is in the frontal right mid to anterior gyrus/lobe running somewhat linear. No apparent paranchymal loss of volume...

Then concluses this may be areas of...or...conclusion.

Very confusing.

Hes my son, hes not a car 😥 we bought with a possble two fold fault, I want to all everything right down to what cells are dysplastic and where and HOW!

Our Paediatric Dr has rushed me off the phone as Ive tried to enquire why the Neurologist re- evaluated his MRI to conclude secondly "more likely" FCD type 2b and congential, based on what? 🤔 - yet she disappears away without any answer for us.

Our son also has severe GDD and is statemented/ Soecial Needs, Severe ASD also. Non verbal non communicative, he is 4 yrs old. I need answers and cant just take these header style conditions they seem to think will be enough to settle a parents curiosity.

I’m sorry to hear that the previous reply was deleted!

HIE will show more diffused imaging on the MRI, but this is very difficult to say, as there might be a slight difference across MRIs, depending on patients. However, diagnosis cannot be solely made on MRI scan, as the hospital will need to look further into other factors such as CT scan, ECG (For heart activity), Blood test, EEG (baby’s brain activity), ultrasound scan.

However, for FCD type II, it is easier to diagnose with MRI. It will require other tests to confirm the diagnosis, but it is generally easier to diagnose with MRI when compared to HIE. For FCD, the MRI is seen with thickening of gyrus/ blurring of grey-white matter junction. The latter sign is also found on MRI of the HIE, which makes it more difficult.

I am very sorry to say that I cannot provide you with a full medical knowledge since I am not a doctor myself, but hopefully, this makes more sense to you.

Very interesting information.

I asked for a meeting with the Neuroradiologist and and it was as if I spat at our Paediatrician, how dare I want answers in more depth than they were prepared to state...

Im still waiting and in for a long wait.

I just dont understand why they missed all this on our sons 6 day old MRI.
