has anyone pulled together with their GO a Heart Failure Care Plan ? If yes can you kindly share the format used, not the context please, be very helpful Inhave a call with my GP this coming Monday thanks

Hi, my Dad was diagnosed with Heart Failure 13 yrs ago. After the initial tests etc. his care transferred over to the specialist Nurses at our local H.F Clinic. The G.P has very little to do with his cardiac care. The Nurses prescribe all his meds (and usually distribute the first packet there and then). They then advise the G.P of their actions and those meds get added to his regular prescriptions. They organise any scans etc. that may be needed and can carry out home visits if necessary. In fact, when my Dad was in A & E on one occasion, the Doctor there deferred to the H.F Nurses medication expertise - as he recognised cardiac care wasn’t his speciality. They can also ‘task’ other medical professionals that may need to be involved with Dad’s care - whether that be District Nurses, the local surgery etc.

So assuming you are in the U.K - I suspect every N.H.S Trust will have a H.F Clinic that you can be referred to and then remain under their care. Hope you get it sorted.

Thank you so much for getting back to me and with such a comprehensive response. I am in the UK and it will be interesting to see what my GP has to say. I live in a semi rural area the specialist team that treated me is approximately 35 miles away, there is a more local hospital with limited resources. I will update my post once I have spoken with my GO. Thanks again
