I have found my Apple iWatch very reliable and it would do everything on your list. Of course you also need an Apple iPhone so it is not a cheap option.

Hi, I have a Withings Scan watch, they are a little pricey but approved by cardiologists in uk and usa for their ecg recordings. It links to an app on your phone which you can then pdf the results and send to your Dr. Can also monitor heart rate, steps exercise. Its not constant monitoring, you need to start a reading, but has been very useful to provide my cardiologist with evidence when I am having eptopic beats etc.

My Samsung watch6 seems pretty good when I compare it to the oximeter and blood pressure sensor I bought.

This is for heart rate not ecg types of readings.

I use a Polar H10 chest strap. The strap can connect to your phone via the app and can also connect to most gym equipment to monitor your heart whilst working out.
