If your doctors think you need one they will arrange it

Not necessarily, sometimes they need a nudge. Under NICE guidelines, if you have HF then you should be referred to a nurse. My husband wasn't referred until we brought up the issue with an ICD support clinic run by Papworth, they then got in touch with the nurses & the first thing the nurse did was change his medication.
