You can view it from either patient or provider perspectives to gain information. Plus it will keep track on a calendar how you are getting on, on a day to day basis. It has a wallet card to keep your vital medical information, especially useful if you see more than one physician. And another page provides questions you might want to ask your caregivers or make note of your own questions to ask.
