although I use MART I have found that the plan to increase my inhalers has not resulted in avoiding an exacerbation of my asthma on the last two occasions. I am due to review my plan next week

Honestly I don’t know if I have a plan - I’ve been told I can increase the normal dosage when needed, but that’s all…

Me too sadly .x

My 6 month MART trial was unsuccessful and it made my asthma worse. But I don't tolerate high amounts of LABA and/or steroid. Asthma flares are treated with Azithromycin rather than Prednisolone.

I settled on MART while reducing my inhaler usage, but then got a call from a different nurse at the surgery telling me to use Ventolin instead 🤷🏻‍♂️

I have used MART in the past but am now on Relvar so limited to 1 dose per day and back to a separate reliever.

I am on daily Relvar Ellipta 184 mcg and Ventolin PRN

I’m on Relvar 184/22 too. You can’t use the MART system on Relvar.

Do you get on ok with relvar?

Yes Relvar 184/22 has been brilliant for me - kept everything under control really well. I have an exacerbation at the mo due to this nasty cold virus that’s been going around over Christmas but it’s the first time in 18months that I’ve needed pred. So easy with just 1 puff a day too. Game changer.

I have been on Relvar 92/22 since 2011 and only changed to 184mcg following a bout of pneumonia late 2023. It really works for me, I haven’t had any issues.

Ok, thank you, do you know why?

I have been on MART regime for a number of years for COPD, Emphysema and an asthma overlay. When I get any chest infection, I am told I can take up to 8 extra puffs if needed throughout a day. However, I still take one puff of Ventolin in the first instance.

I've never heard of the term "MART" inhaler. I'm on Trimbow twice a day , which I'm sure is a combination inhaler, but have never been told about using a plan.

I also have Salbutamol to take when required

Poobah profile image

Trimbow isn't a suitable inhaler for MART (maintenance and rescue treatment) as it contains 3 medicines, one of which cannot be increased above the prescribed daily dose. Other inhalers that can be used for MART contain 2 medicines; an inhaled steroid and a long acting bronchodilator. So instead of using Ventolin or Salbutamol for a rescue inhaler, the MART inhaler is used as a rescue inhaler, up to a maximum number of inhalations a day. This is in addition to the normal/regular morning and evening doses.

Oh thanks for that information Poobah.I feel my asthma is not very well managed .

I don't know if the Trimbow is to help with my bronchiectasis.

I often get confused whether it's my bronchiectasis or the asthma making me breathless.

I've noticed I'm nebulising Salbutamol more frequently atm .

Must speak to my consultant about the MART inhaler, thank you .

It is possible to split the 3 medications in your Trimbow into 2 inhalers. So you could take the Glycopyrronium type med in one inhaler (LAMA inhaler) and the other 2 medications in another inhaler (LABA inhaler) that could potentially be used in MART, if your consultant thinks you're a suitable candidate.

But if Trimbow isn't controlling your symptoms, you should make that clear to the consultant before mentioning MART. And say you'd like to try a different approach in order to achieve better control, so what are your options? You can raise MART in the subsequent discussion about your treatment options. Never hurts to steer the conversation.

All the best.

MART= maintainance+ reliever therapy= using steroid inhaler as a preventer and reliever

A steroid inhaler with a long acting bronchodilator. 😊 A steroid inhaler wouldn't give any short term relief/rescue.

Never been on the MART plan/inhaler.... I have had RAD and asthma for more than 50 years. Interesting concept.
