If you take one of the DOACS then a card usually comes in the pack. I made and printed my own with my photo and other essential medical information on. You can also look at medical alert bracelets etc. I also make sure all my information is on my medical alert on my phone.

I have a new card with every box of Predaxa and when I was on Warfarin I was given a yellow record book by the hospital/GP
There is a card in every box of apixaban.
As others have said there is a card in the meds. I only have 2 at the moment but as I accumulate more I'll keep one in my pocket, wallet, handbag etc.

Normally the alert cards come with the anti-coagulants if you take a DOAC, but I did find these a bit of a nuisance putting one in every pocket and handbag, etc (although I still do so). I applied for an identity bracelet and I found a website who are not only quick, but also quite reasonable plus they allow 4 lines on the front and 4 lines on the back of it whereas a number of them only seem to allow 2 lines.
