Unexpected early end to ADT
As I wrestled with this decision, the thought of getting back to who I was before the
ADT
started seemed pretty damn good! And I am motivated and excited to rebuild my body and mind. Thoughts?

Pluvicto damaging salivary glands?
Now on
ADT
holiday since 1-1-24, psa unmeasurable so far. During the treatment, had increasingly serious drymouth and nasal congestion and irritation with bleeding, which I attributed to radiation damage. Since then, there has been some, slow improvement, but still not right yet.

At the crossroads (again)
My profile is up to date, but, briefly, have failed chemo,
ADT
(am castrate resistant), failed Keytruda and now Pluvicto. Scans yest. & today show progression along with PSA rise. All my mets are in bone. So, my SOC choices are cabazataxel or radium 223.

Scary PSA results
Started
ADT
on November 9. Did 28 sessions of IMRT from Feb 12, 2024, to Mar 21, 2024. Nadir was .8 on Mar 26 and then the last two have been 1.5 and 3.1. RO said 2.8 was the "something more needs to be done marker" I am seeing MO next week. What questions should I be asking?

Should the dose of Dutasteride be increased?
STEP 2: Add Dutasteride to
ADT
. We know that using the standard dose of 0.5 mg/day of Dutasteride reduces DHT by 94% (reduction factor = 16.7 X). I believe that this 94% reduction will synergistically magnify the % reduction from doing Lupron
ADT
.

Stage 4. IMRT + HDR Brachytherapy
Currently on
ADT
. I have chosen IMRT followed by HDR Brachytherapy. I'm starting IMRT in January 2024. What was you experience with this combination? Did it work? Any side effects? Thank you for your responses.

Guess what? Your body has changed and will never be the same.
ADT
makes it harder so, you have to suck it up, put on your big boy pants and work boots and get at it!

Age and timing for Triplet therapy?
I started
ADT
with Ogovyx 4 weeks ago and feel no side effects so far. The urologist has set up the addition of the ARI Erleada in 3 weeks, though I would now prefer Nubeqa because it does not cross the blood-brain barrier.

Another brother passes
Failed
ADT
, chemo and Pluvicto. He was not on this forum. He was very active, skied 120 days a year, biked the rest. He and his wife went to Europe every year. One year to ski and the next to bike. He lived a full life, and that is all we can ask.

6 months after MRI Guided SBRT at MSK
My testosterone recovery was pretty rapid and the
ADT
symptoms started to clear about a month after I finished my last pill. I had my post-treatment labwork two weeks ago which was six months after radiation treatments and four months following the end of
ADT

ADT for almost 2 years
Well, I have gone through surgery, radiotherapy,
ADT
, since my diagnosis in February 2018. It will be 2 years in February next since I started
ADT
and PSA has always been <0.01, but a couple of day ago it came at 0.03.

Rising PSA
ADT
on and off. Recently while on Lupron & NUBEQA PSA continues to rise. Now at 10.1 and Testosterone nondectable. PSMA/PET/CT scan last weeks states no metastases present. We meet with Dr. Aggarwal next week.

PROTACs: another technique to help with mCRPC
I always wondered, if these protein degraders can basically destroy AR receptors, hence overcome both castration resistance and making the cells refractory to androgens...wouldn't it be useless to continue with
ADT
? Oh well... https://www.sciencedirect.com/science/article/pii/S0753332222015013

EMBARK Study: ADT vs Enzalutamide +/- ADT
It compared high risk BCR PCa treated in 3 groups: Lupron
ADT
alone. Enzalutamide plus
ADT
. And Enzalutamide monotherapy. 5 year follow up for metastasis free survival. Results below. Interesting that Enza monotherapy (No
ADT
) was superior to
ADT
and nearly as good as the combined treatment.
