T4 low TSH normal

Evening, I've had more blood tests done and I'm fed up of feeling so badly every day. I've done medichecks TSH tests and cortisol. Only test low is my T4 which tbh never changed since 2016 TSH fluctuates. I had telephone appointment before my test results came back. Due to my chronic fatigue. Weight gain. Constantly aching limbs. Especially my legs ,No strength. I thought that me being on b12 quarterly I'd feel better after all these years but I just feel awful. Doctor said folate was a bit low. Now I have the results. Can I maybe have some guidance on how to approach doctor and take me seriously 😑

My cortisol was

191 nmol/L

You need FULL iron panel for anaemia including ferritin

Also folate, B12 and vitamin D

What vitamin supplements are you taking

Ft4 is below range

You need referral to endocrinologist

Hi slow dragonB12 injections quarterly. Folate. Vitamin d and folate drop below range all the time.

B12 injections can be/may need to be more frequent

As you have B12 injections it’s recommended also to supplement a good quality daily vitamin B complex, one with folate in (not folic acid) may be beneficial.

This can help keep all B vitamins in balance and will help improve B12 levels between injections

B vitamins best taken after breakfast

Thorne Basic B recommended vitamin B complex that contains folate, but they are large capsules. (You can tip powder out if can’t swallow capsule)

Thorne currently difficult to find at reasonable price, should be around £20-£25. iherb.com often have in stock. Or try ebay

Igennus B complex popular option too. Nice small tablets. Most people only find they need one per day. But a few people find it’s not high enough dose

IMPORTANT......If you are taking vitamin B complex, or any supplements containing biotin, remember to stop these 7 days before ALL BLOOD TESTS , as biotin can falsely affect test results

In week before blood test, when you stop vitamin B complex, you might want to consider taking a separate folate supplement (eg Jarrow methyl folate 400mcg) and continue separate B12

A low fT4 with a non-elevated TSH suggests secondary hypothyroidism cks.nice.org.uk/topics/hypo... . Get your doctor to refer you to a competent endocrinologist. You’ve had results like this for quite some time, they need to stop messing about.

Your cortisol results are also very low, near borderline which suggests looking at Pituitary being the problem. Have you had any type of head injury in the past as this can cause Pituitary failure? Look for an Endo who has experience of Pituitary problems, recommendations and help from Pituitary Foundation. I have pituitary failure and it's taken me 20 years to find someone who understands. Update from GP.Highly unlikely I have thyroid issues if my TSH normal. I said I feel awful I mentioned that I was dismissed when I felt so poorly and through my own pushing of private blood tests my b12 was around 57 low folate and vitamin d was 20.

I said the 3 monthly injections I don't feel better for it. And the doctor said well that wasn't the problem then.

She didn't offer to do any bloods. Said what was I wanting from this appointment.

She asked if I had depression and when was my last period and did I still have a uterus. She pushed me towards menopause. 😞 Don't know where to go now really.

Don't give up 🤗 you know what the issue is, you just need to find a competent Endo as GP's aren't knowledgeable enough to go beyond TSH levels... did you discuss a referral with your GP, it is up to you to push for this as they won't!
